Hello Aaron

I really need an Efficient plugin to display related posts. But my blog uses only categories and I am afraid of changing them to tags due to permalinks and stuff. So I wanted to ask if efficient related posts plugin works with blogs that have only categories and no tags. Will it then match categories to display related posts? 

Or is there some workaround which allows me to use the plugin without changing anything like categories, permalinks and stuff?

It will only work if you use tags.  However, you don't have to stop using categories in order to use tags, and you definitely don't have to change your permalinks at all.
